This application from must be submitted online by Friday 14 November 2008.
Note: Your application form will be sent automatically to the Graduation Office.
Your name will be printed on your certificate exactly as it appears in the University's student record system. If you wish to change your name you must do so on the appropriate Change of Name form available from the Enrolment Services. Name changes will not be accepted without proof. This must be done at least four weeks prior to the Graduation Ceremony. Name changes made after this will incur a $50 charge if the parchment needs to be reissued.
All correspondence will be sent to your mailing address as recorded in the student record system. You must keep the address up to date or you will not receive vital information regarding the ceremony or post-graduation information. If you will be returning to an overseas home address please change this to your mailing address at the Enrolment Services.
