Graduation Ceremony Application form

  • December 2017 Graduation Application Form

  • This form is to be used for the application for the Conferring of Degree, or Award of Diploma or Certificate at the next available Graduation Ceremonies. Please refer to the Graduation webpage for full details and dates of the next available ceremonies.

    Please ensure you are aware of the eligibility criteria for the December Graduation Ceremonies. Full details can be found on the Graduation webpage.

    This application form must be submitted no later than 4 weeks prior to the Graduation Ceremonies. Please refer to the Graduation webpage for the designated closing date for applications to attend the next available ceremony.

    Note: Your application form will be automatically sent to the Graduation Office, you will also receive a copy by email to your nominated email account. This form is to be completed if you wish to attend a graduation ceremony.

    If you are a Darwin based Doctor of Medicine student attending the NTMP Graduation Ceremony do not use the form below. Please click on this link to access the Darwin NTMP Graduation Ceremony form.

    If you do not wish to attend a graduation ceremony, do not proceed with this form. Please use the Absentia conferral form to apply to have your degree conferred in absentia.

  • PERSONAL DETAILS

  • NAME ON CERTIFICATE

  • Your name will be printed on your certificate exactly as it appears in the University's Student Information System. If you wish to change your name you must do so via Flinders Connect by submitting an 'Ask Flinders' request. This must be done at least four weeks prior to the graduation ceremony. Name changes made after this date will incur a fee of A$50 if the parchment needs to be reissued.

  • PHONETIC NAME

    Your full name will be read out as you cross the stage at the ceremony. If you believe the Reader may require assistance with the pronunciation, please write your name, in full as it sounds below: (E.g. the phonetic spelling for Stephen Damon Smith is " Stee-ven Day-man Smith ").

  • ELIGIBILITY TO ATTEND

  • To be eligible for the December Graduation Ceremonies, you must meet certain criteria. Those students who are eligible to attend are listed below.

  • CEREMONY REQUIREMENTS

  • DEGREE/DIPLOMA/CERTIFICATE DETAILS

    (eg Bachelor of Science, Master of Nursing, Graduate Diploma in Legal Studies)
  • CEREMONY GUEST TICKETS

  • Guest tickets are complimentary and can be requested below. Up to a maximum of four (4) tickets are permitted per graduand for family and friends. All babies and children require a ticket. There is only limited space for prams and strollers. Graduands do not require a ticket.

  • You may request additional tickets in the box below, please be aware that the allocation of additional tickets is not always possible for every ceremony. Should any additional tickets become available, you will be notified in your confirmation email which is sent seven (7) days before your ceremony.

  • HAND SHAKING

  • It is common practice for graduands to shake hands with the Ceremony Officiator at the ceremony on stage. If you, for religious or personal reasons, would prefer NOT to shake hands with the Ceremony Officiator, please indicate this by ticking the box below:

  • SPECIAL REQUIREMENTS

  • ACADEMIC DRESS HIRE

    In order to participate in a graduation ceremony all graduands must wear the correct academic dress.

    If you have selected to provide your own gown or hood, you may be required to provide photos of the items so that they can be verified as meeting the requirements that have been stipulated by the University. Your hood colours are determined by your award and must be adhered to.

  • Indigenous Stole: A person who identifies as an Aboriginal or Torres Strait Islander Australian is entitled to wear a stole, worn over the gown and under the hood of the academic dress for the relevant degree.

  • CONTACT DETAILS

  • All correspondence will be sent to the email address nominated below. You must keep all contact details up to date or you may not receive vital information regarding the ceremony or post-graduation information.

    If your email contact details should change after you have submitted your application, please contact the Graduation Office.

  • By submitting this application you acknowledge that this event is being filmed and screened across multiple locations. Graduation footage is also available on the website after the ceremonies have taken place. If you do not wish to be filmed, please contact the Graduation Office at graduation@flinders.edu.au and we will make every effort to accommodate your request.
