To register for the Flinders Chronic Condition Management (CCM) Program™ click on the link below. Complete the registration form and email to CCM@flinders.edu.au or fax (08 8404 2101) or mail to FHBHRU, Margaret Tobin Centre, Flinders University, GPO Box 2100, Adelaide SA 5001.

Workshop Cancellation Policy (PDF 47KB) In the unlikely event that a workshop has few enrolments we will give you two weeks cancellation notice.
FHBHRU CCM Accredited Trainers around Australia and in some overseas countries also offer Flinders CCM Program workshops.

Chronic Condition Management Refresher (1 day)

This one day workshop is aimed at health professionals who have attended a CCM training in the last 3 years without passing all assessment for a Certificate of Competence. Participant’s experience is considered in the tailoring of this workshop and assessment necessary to gain a Certificate of Competence. Chronic Condition Management Trainer Accreditation Workshop

- 31 July - 1 August 2012
CCM Trainer Accreditation (with pre-requisites*) is a 2-day workshop with assessment requirements to equip health professionals to deliver the CCM Program. Some Accredited Trainers also contribute to the Closing the Gap and Coordinated Veterans' Care programs run by FHBHRU.

Who can apply to attend the Trainer Accreditation Workshop?
You are eligible to undertake our CCM Trainer Accreditation training in Chronic Condition Management if you meet the following prerequisites:
- Attended a Flinders Human Behaviour & Health Research Unit Chronic Condition Management Workshop (2 day);
- Obtained the Flinders Human Behaviour & Health Research Unit Certificate of Competence in Management by completing three Patient Assessments and Care Plans, using the Partners in Health Scale, Cue & Response Interview and Problems & Goals Statements and faxing (or mailing) your completed, de-identified forms to the Flinders Human Behaviour & Health Research Unit;
- Have a Certificate IV in Training Assessment or equivalent.
