Minor Plant Risk Assessment

The following checklist is a LOCKED document so that you can record your response by 'clicking' on the appropriate box or pull-down menu.
BUT When you need to enter text you must UNLOCK the document first.

To unlock the document follow this path: on the menu bar select View>Toolbars>Forms toggle the padlock on the far right of the toolbar to unlock/relock the document.
