Authority forms: Confidential files

In order to enable exchange of information relating to the staff member's injury/illness the Rehabilitation Case Manager will request that the staff member sign a form authorising exchange of such information.

Any information which concerns medical, family, financial or emotional matters is considered to be 'personal information' and is kept strictly confidential.

The authority form and all personal information is kept on the staff member's rehabilitation file and the file is kept secure by the Rehabilitation Case Manager.

The rehabilitation file will contain detailed records of the rehabilitation and return to work process and a copy of the rehabilitation outcome will be placed on that file.
