Injured staff member
The Rehabilitation Case Manager will meet regularly with the injured staff member to monitor medical progress and capacity to return to work either to normal duties or modified duties/hours.
The injured staff member will have regular medical reviews and obtain updated medical certificates, which will assist the Rehabilitation Case Manager to determine if suitable duties can be provided. If progress is slow, the Rehabilitation Case Manager may initiate a case conference with the doctor and the worker, with the aim of facilitating better communication as to the extent of the injury and the nature of the duties being provided.
A rehabilitation Program Plan will be developed in the early stages whilst the staff member is still in the medically intensive phase, with the goal of progressing to a Rehabilitation and Return to Work Plan. Once the Return to Work Plan is in place, a gradual increase in duties/hours is expected and at least fortnightly reviews will take place.
Supervisor
Where a rehabilitation and return to work plan is proposed/revised
the Rehabilitation Case Manager will discuss the proposal
with the supervisor and obtain his or her agreement and signature
on the rehabilitation and return to work plan. A copy of the
rehabilitation and return to work plan will be provided to
both the injured staff member and the supervisor.
Prior to a return to work the Rehabilitation Case Manager
will contact the injured staff member's supervisor after each
discussion with the injured staff member, and inform him or
her of progress and projected time frames.
