Subsidy for glasses
Subsidy for glasses under the university's screen based
equipment policy
The University will subsidise glasses needed by staff members
who use a Visual Display Unit (VDU) at work provided the glasses
are specifically prescribed for their VDU work, either because
the visual problem arises with VDU work and the glasses are
used only for that work, or in the case of staff members who
have glasses for other purposes, there is a need for different
glasses for VDU work. Screen

Reimbursement Rates: $60.00 single vision, $115.00 multifocal
If you are submitting a claim under the above policy please
supply the Occupational Health and Safety Unit with the following:
- A certification form,
completed by the Optometrist/Ophthalmologist.
- The receipt (original) from the optometrist.
- If you are claiming from private health insurance, please
provide an (original) payment slip.
- Place all the above documents in a 'Staff Expenses
Claim Form' envelope, (available from accounts offices).
Write your name, department and staff ID number which is
printed on your payslip and staff card, sign and date under
'Advance Holder' section.
