State and Territory legislation requires the University to notify the relevant State or Territory authority of certain accidents and incidents.

South Australia

Under the South Australian Occupational Health Safety & Welfare Act, 1986, the University must notify SafeWork SA within 24 hours any

  • Work related death
  • Injury that
    • requires admission to hospital as an inpatient; or
    • is from exposure to any substance that causes acute symptoms
  • Dangerous occurrences

Dangerous occurrences include

  • The collapse, overturning or failure of the load-bearing capacity of lifts, cranes, hoists or scaffolding;
  • The damage to, or malfunction of, other major plant or equipment
  • Unintended collapse or failure of an excavation more than 1.5meters deep
  • The collapse of a floor, wall or ceiling of a building used as a workplace
  • An electrical short circuit, malfunction or explosion
  • An uncontrolled explosion, fire or escape of gas, steam or other hazardous substance
  • An unintended event involving a flood of water, rock burst, rock fall or collapse of ground
  • Malfunction of breathing apparatus to the extent that the user's health is endangered.

The OHS Unit is responsible for reporting any such accidents/incidents to SafeWork SA. Therefore you must report immediately any such accident or incident to the OHS Unit (13024) during business hours or to University Security (12880) after hours.

The OHS Unit must also notify the Office of the Technical Regulator, within 24 hours, of any accident or incident that involves electric shock as well as any incident involving electricity that might have caused injury.

Hence and accident or incident involving electricity on University premises must be reported to the OHS Unit as soon as possible after the event.

Victoria

Under the Victorian Occupational Health and Safety Act 2004 and the Equipment (Public Safety) Regulations 2007, the University is required to notify WorkSafe Victoria of any serious workplace incidents immediately they occur, and to preserve the incident site until an inspector arrives or directs otherwise.

The OHS Unit is responsible for reporting any such accidents/incidents to WorkSafe Victoria. Therefore you must report immediately any such accident or incident to the OHS Unit (08 8201 3024) during business hours or to University Security (08 8201 2880) after hours.

Northern Territory

The Northern Territory Workplace Health & Safety Act requires the University to notify NTWorkSafe: Incident Notification of any

  • Work related accident/incident where there has been a death or serious injury or there was a potential for this to occur;
  • An incident at a workplace creating a risk of a work-related accident and consisting of
    • A major structural failure or collapse; or
    • An explosion, implosion or fire; or
    • The escape, spillage or leakage of a harmful or potentially harmful substance; or
    • The falloff an object from a height; or
    • The failure of a system on which the health or safety of workers is dependent (eg diving apparatus)
  • An electric shock at the workplace; or
  • An incident classified by the Regulations as a reportable incident.

The OHS Unit is responsible for reporting any such accidents/incidents to NTWorkSafe. Therefore you must report immediately any such accident or incident to the OHS Unit (08 8201 3024) during business hours or to University Security (08 8201 2880) after hours.
