Topic description
This topic addresses the management of chronic conditions from a population health perspective and with an emphasis on self management. The topic will examine the international and Australian evidence for the effectiveness of various models of care for chronic physical and mental conditions, the current Australian health system and practices of managing chronic conditions, and, the role and models of self management.
Educational aims
1. The topic aims to provide each participant with the opportunity to develop knowledge, skills and confidence to:

2. Identify the current approaches to the management of chronic conditions in Australia and Internationally

3. Have knowledge of the different Population Health and the Public Health approaches to chronic condition management

4. Apply the concepts of population and public health to disease management

5. Critically analyse past and present models of chronic condition management from a systems perspective as they are used in the Australian and international context

6. Identify the role of evidence-based medicine in chronic condition management
Expected learning outcomes
Students successfully completing this topic will be able to:

1. Examine and explain the link between Population Health evidence and chronic disease management both in Australia and Internationally

2. Understand and explain the value of using a Population Health approach to the management of chronic conditions

3. Identify and critique the different approaches to chronic condition management

4. Identify the implications for chronic condition management and self management from a range of perspectives, across service boundaries and levels of health service provision

5. Apply their understanding to collaboration in the development of a proposal for health practice change within their practice which supports effective chronic condition management within healthcare systems
