Topic description
Students will have the opportunity to synthesise the learning they have achieved in other topics and apply this to a specific health condition, context or population of their choice with a view to generic issues. Conditions may include, as examples, asthma, diabetes, arthritis, specific mental health diagnoses, a specific addiction, chronic obstructive airways disease, chronic pain, and so on. Either acquired or congenital conditions may be chosen. Students will examine, in detail, the specific needs and challenges of applying a generic chronic conditions self-management approach to their chosen area of focus, with consideration given to individual, community, and systems implications as these relate to existing health and welfare policy for the area of focus.
Educational aims
1. To build on and synthesise the learning achieved in other generic topics on CCM and CCSM and apply this to specific health chronic conditions (for example: cardiac, respiratory, mental health, cancer, diabetes, arthritis, chronic pain), contexts or populations (for example: early intervention/outpatient/inpatient settings, mental health settings, aged care, indigenous populations, culturally and linguistically diverse (CALD) populations, non-government organisations, rural and remote settings, specific organisational settings, specific genders, or addictions rehabilitation settings).

2. To understand how chronic condition management and self-management apply to specific conditions, contexts or populations, as well as how organisations and systems that provide support services to these groups respond.

3. To examine, in detail, the specific needs and challenges of applying a chronic conditions self-management approach to specific conditions, contexts or populations.
Expected learning outcomes
Students successfully completing this topic will be able to:

  1. Design and develop strategies that promote better CCM and CCSM for specific issues within health organisations, based on a clear knowledge and understanding or current practices, issues and challenges.

  2. Implement effective programs for health professionals and service consumers with regard to specific health conditions based on a sound understanding of the research evidence and practice literature.

  3. Achieve a comprehensive understanding of how to apply effective self-management support at the individual, community, systems and policy level for specific conditions, contexts or populations.

  4. Consider and demonstrate ethical practice in understanding the diverse health and psychosocial needs of specific groups within the community.

  5. Effectively synthesise current theory and practice knowledge in their chosen area of focus.
