Assumed knowledge
Students will be expected to have an undergraduate qualification and experience in a health science or health-related discipline (including from medicine, nursing, allied health and the welfare sector). Students will be expected to have current or recent experience of working in the health, welfare, rehabilitation or disability sectors involving the management of, or providing support to people with chronic physical and/or mental health conditions.
Course context
This topic is designed for students in the health, social and welfare sectors and related professional disciplines by enabling a holistic understanding of a variety of client-centred health behaviour change initiatives specific to chronic condition self-management and self-management support. In the context of other topics within the Graduate Certificate and Graduate Diploma of Chronic Condition Management, this topic offers a specialised understanding of the applicability of internationally recognised theories, models and tools for implementing, guiding, conducting and appraising Motivational Interviews with people living with chronic conditions to support health behaviour change.

Topic description
This topic offers students an advanced understanding of the principles and skills involved in the development and implementation of evidence-based client-centred behaviour change strategies and models in diverse practice settings. Successful completion of this topic will enable health professionals to adopt practices that will enhance their ability to support people with chronic conditions to prepare for and take action on behaviour change, and to develop knowledge, capacity, skills and attitudes needed to work more effectively with this client group. This topic fits with the Chronic Condition Management graduate courses by meeting the extended professional development needs of graduates involved in the planning, development, implementation and evaluation of policy and program initatives, targeting the delivery of services to people with chronic physical or mental health conditions.
Educational aims
The topic aims to provide each participant with the opportunity to develop knowledge, skills and confidence to:

  • Apply skills in critically evaluating a variety of internationally recognised client-centred health behaviour change models and theories and their underpinning principles and processes.

  • Translate these models into effective and sustainable clinical practice, improving individual and public health outcomes related to chronic conditions.
Expected learning outcomes
On completion of this topic, students will be expected to be able to:

  • Analyse and compare internationally recognised health behavioural change theories and models suitable for people with chronic conditions;

  • Appreciate the impact of the person’s internal and external resources on their ability and willingness to engage in behaviour change;

  • Explain the underlying principles and processes of Motivational Interviewing;

  • Demonstrate collaborative problem identification and goal setting skills with people living with chronic conditions through motivational interviewing techniques in practice.
