Topic description
This topic provides an opportunity for students to integrate theory and practice in both acute and community healthcare settings and develop skills in patient-centred integrated care. It includes professional experience placement.
Educational aims
The aims of this topic are to:

  • apply and integrate the knowledge and skills learned in previous topics to promote developing nursing practice across a range of healthcare settings

  • become familiar with the ways in which patient data is communicated in different healthcare settings.

Expected learning outcomes
On successful completion of this topic students will be able to:

  • demonstrate safe, effective and evidence-based integrated nursing care

  • coordinate and prioritise nursing care for multiple allocated clients/ patients in an efficient and timely manner (assessment, intervention, re-assessment, planning, documentation)

  • identify and communicate (verbally and written) client/ patient health care needs and advocate in achieving optimal outcomes

  • adapt their practice to differing practice situations and contexts taking into account the role of families/carers/community and the setting in which the care is provided, such as a home or clinic

  • identify the use of informatics in a range of clinical settings, and explain how these are used to facilitate health care for better outcomes

  • perform nursing care within their scope of practice and with the appropriate level of supervision

  • manually perform drug calculations to 100% accuracy.
