Assignment(s)

This topic provides an opportunity for students to integrate theory and practice in both acute and community healthcare settings and develop skills in patient-centred integrated care. It includes professional experience placement.

The aims of this topic are to:

apply and integrate the knowledge and skills learned in previous topics to promote developing nursing practice across a range of healthcare settings
become familiar with the ways in which patient data is communicated in different healthcare settings.

On successful completion of this topic students will be able to:

demonstrate safe, effective and evidence-based integrated nursing care
coordinate and prioritise nursing care for multiple allocated clients/ patients in an efficient and timely manner (assessment, intervention, re-assessment, planning, documentation)
identify and communicate (verbally and written) client/ patient health care needs and advocate in achieving optimal outcomes
adapt their practice to differing practice situations and contexts taking into account the role of families/carers/community and the setting in which the care is provided, such as a home or clinic
identify the use of informatics in a range of clinical settings, and explain how these are used to facilitate health care for better outcomes
perform nursing care within their scope of practice and with the appropriate level of supervision
manually perform drug calculations to 100% accuracy.
