This topic uses a variety of case studies across the lifespan in a variety of cultural settings to stimulate students’ exploration of issues and concepts associated with patients with multiple physiological, psychological and social co-morbidities who are discharged after an acute episode to a community or extended care setting. The topic considers the assessment, planning, implementation, evaluation and documentation of integrated care required for these patients/clients.

The aims of this topic are to:

consolidate understanding of the needs of people with complex health care needs who are discharged from acute care into the community or transferred to an extended care facility
develop the use of information technology to access and use evidence-based information to facilitate clinical problem-solving.

On successful completion of this topic students will be able to:

Use knowledge of pathophysiological, psychological, cultural, spiritual and social processes to describe the needs of people with long-term health care needs in a community or extended care setting
Describe pharmacological issues associated with the deteriorating body
Demonstrate a knowledge of how genetic variation in the population can impact on medication management
Conduct an advanced and comprehensive health assessment of a person with complex needs
Access evidence-based information through various IT mechanisms and incorporate it into advanced clinical problem-solving.
