This topic aims to provide the foundation knowledge and skills in advanced gerontological nursing practice in various care settings. The content covers comprehensive holistic care for older people with complex health conditions, including pathophysiological and psychological changes in relation to these health conditions.

The aim of this topic is to facilitate students to apply knowledge and skills to clinical care for older people with complex health conditions in various care settings; to improve their quality of care and quality of life, and work in partnership with other health professionals, care workers and informal caregivers to promote health and prevent complications for older people.

On successful completion of this topic students will be able to:

explain pathophysiological and psychological changes in older people with chronic conditions and comorbidities
identify risk factors contributing to comorbidities such as falls, delirium, pressure injuries, malnutrition and incontinence
identify approaches and skills to managing chronic conditions and preventing acute episodes
work with a multidisciplinary team, care workers and informal caregivers in managing health conditions, rehabilitation and improving care outcomes and quality of life for older people
apply rigorous research evidence to their own practice to improve care outcomes for older people with complex health conditions
