This topic examines current evidence-based research applicable to the planning, implementation, management and sustainability of care for chronic physical and mental health conditions from a population health perspective. The topic has an emphasis on a variety of Australian and internationally recognised theories, models, tools and processes for implementing chronic conditions management and self-management support initiatives into health services.

The topic aims to provide each student with the opportunity to develop knowledge, skills and confidence to apply current Population Health and Public Health approaches and chronic condition management (CCM) models and theories. This is applicable to improving the fit between the science of implementation and practice change in Australia and internationally to support CCM. The complexity of change management at the macro, meso and micro levels of health service delivery, and Indigenous specific issues and contexts impacting on inbuilt principles and processes will be explored.

On completion of this topic, students will be expected to be able to:

Examine and explain the link between Population Health and models of chronic condition management both in Australia and internationally, and the value of these approaches to the management of chronic conditions.
Critically analyse the barriers and facilitators facing health services attempting to reorient and address the growing demands in the context of people living with chronic conditions.
Identify and critique the different Australian and international theories and models relevant to implementing chronic condition management initiatives into healthcare organisations and systems.
Demonstrate skills for applying the science of practice change implementation within organisations. Explain the principles of sustaining a practice change initiative within their health service supporting effective systems based chronic condition management.
Demonstrate knowledge in Indigenous specific cultural issues and contexts for the application of change management in health services and chronic condition care for Aboriginal people.
