Topic description
This topic explores the care required for patients with a compromised health pattern, and includes:

  • theory underpinning practice

  • activities of daily living (ADLs)

  • rationales for clinical skills

  • practice and development of clinical skills

  • therapeutic communication within the healthcare team

  • the concept of empowerment

  • application of ethical and legal standards

  • patient discharge / hospital avoidance strategies / packages of care / ACAT assessments.

Educational aims
The aim of this topic is to explore and apply the Nursing Process to formulate and identify person-centred care requirements, including multidisciplinary care and consider legal and ethical standards, for patients across the lifespan.
Expected learning outcomes
On successful completion of this topic students will be able to:

  • Apply the Nursing Process to develop an individualised care plan for a patient across the lifespan

  • Determin required care needs to be provided by the multidisciplinary team

  • Prepare all relevant documentation

  • Critically analyse key aspects of an effective discharge plan

  • Explain and apply ehical and legal standards to the care requirements.
