Topic description
This topic uses a variety of case studies across the lifespan in a variety of cultural settings to stimulate students’ exploration of issues and concepts associated with patients with multiple physiological, psychological and social co-morbidities who are discharged after an acute episode to a community or extended care setting. The topic considers the assessment, planning, implementation, evaluation and documentation of integrated care required for these patients/clients.
Educational aims
Develop and illustrate an understanding of the role of the nurse in providing person centered care for people with complex health conditions as they transition from the acute sector to the community or extended care facility.
Expected learning outcomes
On completion of this topic, students will be expected to be able to:

  1. Apply knowledge of pathophysiological, cultural, spiritual and social processes to people with chronic and complex care needs in a community or extended care setting

  2. Examine the role of the nurse in facilitating care associated with medication management for people with complex and chronic health care needs within the community setting

  3. Conduct a comprehensive person centered assessment for the person with complex care needs

  4. Synthesise evidence based Information through various IT mechanisms and incorporate It into clinical problem solving.
