Assumed knowledge
An undergraduate qualification and experience in a health science or health-related discipline (including nursing, allied health professionals and welfare sector). Knowledge of the Flinders model of Chronic Condition Self-Management (CCSM) derived from participation in a Flinders 2 day workshop, including follow up activities. MHSC8111 Managing Chronic Conditions - Self-Management Support Approaches. A Certificate IV in Training Assessment or equivalent.

Topic description

This topic investigates the processes and develops the professional skills involved in education and training about self-management. The topic examines the needs from the perspective of both health professionals and people with chronic conditions, their carers, families and health professionals. Current theories of adult learning, behavioural change and health education program design are included. The topic will include individual learning assessment and program evaluation issues.

Educational aims

This topic aims to:

  • Develop a comprehensive understanding of adult learning theory and practice in order to apply this knowledge to the education and training needs of health professionals, other supports and people with chronic conditions, their carers, families
  • Develop a comprehensive understanding of the process of change, including the barriers and enabling factors associated with attempting to implement change within organisations and specific populations
  • Understand current theories of adult learning, behavioural change and health education program design and apply these theories to the effective design, implementation and evaluation of health education programs
  • Understand and have the opportunity to practice workshop planning, delivery and evaluation within a safe learning environment prior to delivery to the student's chosen community or organisational setting
  • Analyse and evaluate existing health education programs, in relation to self-management, for health professionals, other supports, people with chronic conditions, their carers, families
  • Ensure maintenance of the Flinders Program as a quality product by clarifying the expectations of FHBHRU in the process of training and evaluation, the reciprocal responsibility of
Expected learning outcomes
On completion of this topic you will be expected to be able to:

  1. Effectively communicate and apply a range of learning activities to peers which take into account the needs of adult learners
  2. Communicate the Flinders Program to a range of health professionals: General Practitioners, Allied Health Professionals, Nurses, Aboriginal Health Workers, Consumers, Carers and other support people within a collaborative learning environment
  3. Support health professionals and other support providers to adopt practices that enhance their ability to partner patients/clients to manage their chronic conditions
  4. Independently provide specific skills training to health professionals and other support people to use the Flinders Program tools to form the basis of a self-management care plan with patients/clients
  5. Articulate and apply the approaches and models that will assist health professionals and other support people to identify a patient's/client's readiness and capacity to make health behaviour change
  6. Develop a general understanding of the organisational factors and challenges associated with delivery of education and training in CCM and CCSM support to the healthcare workforce
