Topic description

This topic uses a variety of case studies across the lifespan and cultural settings to stimulate students’ exploration of issues and concepts associated with complex/ chronic conditions in a community. The topic considers the assessment, planning, implementation, evaluation and documentation for the provision of fundamental person-centred care in chronic disease management.

Educational aims

This topic aims to consolidate understanding of complex health care and educational needs and to develop skills to educate clients about self-management of chronic conditions in health care.

Expected learning outcomes
On completion of this topic you will be expected to be able to:

  1. Build on knowledge of pathophysiological, psychological, cultural, spiritual and social processes to develop long term person-centred care plans
  2. Formulate mechanisms to evaluate the self-management knowledge and skills of a person with complex care needs
  3. Formulate mechanisms to utilise technology to provide evidence-based care and promote person-centred self-management strategies
