Topic description

This topic examines the research base for self management and effective chronic condition management. It teaches the use and application of the generic Flinders Chronic Condition Self-Management Program and enables critical appraisal of other internationally recognised approaches. It leads to enhanced skills for clinicians in assessment of self-management capacity and facilitating behavioural change for people with lifelong conditions. It includes consideration of problem identification, goal setting and motivational interviewing in relation to chronic physical and mental health conditions.

Educational aims

This topic aims to provide each participant with the opportunity to develop knowledge, skills and confidence to:

  • Understand the theory and evidence for chronic condition self-management
  • Understand and develop capacity in using the Flinders Program tools for the self-management assessment process and self-management care planning
  • Utilise a range of self-management interventions and strategies
  • Independently and critically evaluate the range of internationally recognised approaches, new ideas and developments for managing chronic conditions
  • Independently translate these into effective and sustainable practice
Expected learning outcomes
On completion of this topic you will be expected to be able to:

  1. Examine the issues facing health services as they address the increasing demands of populations living with lifelong conditions
  2. Demonstrate effective use of communication skills, collaborative problem identification and goal setting with people with lifelong conditions and integrating connection and collaboration with other health workers and support providers
  3. Competently utilise generic self-management care planning tools (the Flinders Program)
  4. Appraise a range of internationally recognised person-centred self-management approaches used to support people with chronic conditions or risk factors for chronic conditions (such as the Stanford Model, the 5 As, peer support, or health coaching)
  5. Independently analyse and evaluate sustainable models for using self-management support approaches in working with people with lifelong conditions
