To build on and synthesise the learning achieved in other generic topics on CCM and CCSM and apply this to specific health chronic conditions (for example: cardiac, respiratory, mental health, cancer, diabetes, arthritis, chronic pain), contexts or populations (for example: early intervention/outpatient/inpatient settings, mental health settings, aged care, indigenous populations, culturally and linguistically diverse (CALD) populations, non-government organisations, rural and remote settings, specific organisational settings, specific genders, or addictions rehabilitation settings).
2. To understand how chronic condition management and self-management apply to specific conditions, contexts or populations, as well as how organisations and systems that provide support services to these groups respond.
3. To examine, in detail, the specific needs and challenges of applying a chronic conditions self-management approach to specific conditions, contexts or populations.
