Topic description
Students will build on the learning they have achieved in other topics and apply this to a proposal for change for the specific health condition, context or population of their choice from Chronic Conditions Management for Specific Conditions, Contexts or Populations A. Students will gain skills and confidence, and be supported, to publish their work for dissemination to the field, based on synthesis of materials from their previous case study and literature review undertaken within their chosen area of focus.
Educational aims
1. To build collaborative learning skills, develop and share expertise in a chosen area of focus related to chronic condition management and self management for a specific condition, context or population.

2. To systematically plan, design and develop a proposal that would enable the further investigation of chronic condition management and self-management for a specific health condition, context of population at an organisational and systems level.

3. To gain confidence and skills in writing for publication and other forms of dissemination of knowledge to peers, researchers and policy makers in the field.
Expected learning outcomes
1. Develop skills in providing constructive peer feedback across diverse, inter-professional boundaries.

2. Apply knowledge pertaining to a chosen area of focus related to chronic condition management and self management for a specific condition, context or population to a proposal for change within the field of practice.

3. Effectively synthesise, communicate and disseminate ideas from current theory, practice knowledge and experience in their chosen area of focus.
