This application should be completed with reference to the Student-Alumni Mentoring Program Participant Guidelines.

Information you provide will be used to help connect you with a suitable mentor.

Any personal information collected is to enable the operation and maintenance of the Program and its services and will be managed as per the University’s privacy policy.

A mentor will only be provided with your name and email address after you have given us consent to do this.
